I am aware that each Reach Yoga Intensive Study Program (herein referred to as the Program) is here to serve me by sharing knowledge of yoga and health. I recognize that yoga requires physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the Program. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in the Program. In consideration of being permitted to participate in the Program, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the Program. I agree to indemnify and hold harmless Reach Yoga LLC, their officers, Lead Teacher Trainers, directors, employees, agents, and volunteer staff against all claims, actions, demands, proceedings, liabilities, cost and expenses, including reasonable attorney’s fees, which they may have ascertained against or incurred by them arising as a result of my participation in the Program. If planning to obtain certification as a yoga instructor with the Yoga Alliance, I understand that I must complete all the 200-hour requirements no later than one year from the start of the program in order to be certified. I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue or make any claims of any kind whatsoever against the Program or any of the aforementioned parties for any injury, property damage/loss, or death caused by their negligence or other acts.

I, the undersigned, agree to the following: 1. A *nonrefundable* deposit of $350 is due upon registration. This deposit is not refundable under any circumstances. For example, if you experience a change in your health or residence, the deposit is not refundable. 2. Photos and/or short video segments that include my image can be used in conjunction with future promotional efforts at Reach Yoga. 3. If you withdraw from the program prior to completion, you may or may not receive a partial refund of tuition you have paid. You must notify us of your withdrawal in writing (by email). We will calculate the value of the program as of your withdrawal date as follows: Hours of instruction elapsed (regardless of your attendance prior to withdrawal) will be charged at a rate of $35 per hour. Your attendance at regularly scheduled yoga classes will be charged at $16 per class. If at time of withdrawal, the amount you have paid (excluding program deposit and any payment plan fee) is higher than the value of the instruction you have received as calculated above, you will receive a refund accordingly.
Complete this section if you are enrolling in the 200-Hour Teacher Training Program. I, the undersigned, agree to the following: The 200-Hour Program takes place between October 2016 and March 2017. All classes, workshops, special classes, and other requirements are to be completed during this period. Participant must complete 90% of scheduled training hours to earn certification. Should participant miss more than 10% of scheduled hours, makeup tutorials will be made available at mutually agreeable times at a fee of $75 per hour. The teacher training faculty reserves the right to grant 200-hour certification based solely on its discretion and evaluation of each student’s readiness to be a yoga instructor. This evaluation is based on successfully meeting the course requirements including, but not limited to, those listed in this document. I understand that if for any reason I do not complete all of the requirements for certification within 6 weeks of the conclusion of the course, I will be required to pay a $26 per month late fee. All requirements including any accrued late fee(s) must be received no later than 12 months after the start of the course. If you do not complete the requirements for the course and submit the late fee within this timeframe, you will not be eligible for certification. If I am not granted certification, I understand that I have the right to appeal to the Reach Yoga faculty, whose decision shall be final.

Complete this form only if you have selected Payment Plan B, Payment Plan D, or another automatic credit card charge. Description of Services This document (Agreement) constitutes your consent to automatic payments for Yoga Intensive Study applied to your credit card. Frequency of Payments Your first payment will be charged immediately prior to the first day of teacher training. Payments will be automatically charged to the credit card you provide, on the payment schedule you indicated in this form. No prior notice will be given, but if you provide an email address, you will get an email informing you when the charge was made. Automatic Recurring Payment Failure If your automatic recurring payment is declined, you will be contacted by phone or email, and you will need to provide another payment method. In the event any payment is not made and you do not make a payment within 5 business days of the due date, your privileges to continue with the Program will be suspended. Cancellation of Automatic Recurring Payment You have the right to withdraw your consent to this Automatic Recurring Payment at any time. If no other payment method is provided, this will constitute a withdrawal from the Program. If you choose to withdraw from the Program at any time, you are no longer responsible for the remaining balance of the Program. In most cases, payments processed prior to Program withdrawal are nonrefundable (see Program Contract for circumstances in which a refund may be issued). Changing Automatic Recurring Payment If you would like to change your Automatic Recurring Payment including, for example, making a change from the credit or debit card initially selected to another credit or debit card, you must authorize the recurring payment with the new credit or debit card at the studio. Your new information will be implemented before the next billing date.
